Developing S2S (Soar To Success) Living: OUR Future Goal

The pilot program is focused around foster or former foster youth ages 16 through 24 who are homeless, potentially homeless or who have run away and need a place to call home. The continuous period will not exceed 18 months.

Sondra's song would like to offer an 18-month S2S Transitional living, a comprehensive spectum of services that provides support to young persons, age 16-24 who need assistance with transition to adulthood. This assistance will be tailored to meet the needs of the individual and may include supported education, work and their housing. Services are person centered, culturally competent and community based.

The Following Crieteria Are The Guidelines For Involvement:

• The person must be between the ages of 16 and 24.

• Each youth must be enrolled in school or maintain at least a part-time job.

• The person must live in West Texas/NM. The person must be a foster or former foster youth. The person must go through an application process for S2S (Soar To Success) transitional living..
